Kindergarten Requirements
In order to enroll in kindergarten at Fort Thomas Elementary, your child must be five years old by September 1. You must be the child's parent or legal guardian in order to register them.
New Student Registration Requirements
Parents who wish to register students new to our district may come by the high school office to register. Our high school office is located at:
15502 W. Hwy 70
Fort Thomas, AZ 85536
Students must live in the district with a parent or legal guardian in order to attend Fort Thomas Schools.
Information Required to Register a Student
You will be expected to furnish the following information:
Birth Certificate
This must be an original or certified copy and must be presented at time of registration. Copies will not be accepted.
Immunizations
Arizona law requires written proof of immunity against certain childhood diseases. Registrations will not be accepted unless immunizations are up-to-date and you have proper records. There are no exceptions. Proof of immunization could include a doctor's medical records or a current immunization booklet or form.
Report Card, Transcript, or Withdrawal Form from Previous School
If you don't have a record of previous grades or placement, a call will be made to the previously attended school for grade and placement verification. For high school students, transcripts from school(s) previously attended are very helpful at the time of registration so that your student will be placed in the proper academic classes.
Tribal Census Number
If applicable, please provide your child's tribal census numbers at time of registration.
Social Security Number
In addition to the above information, transferring students must also provide us with their social security number at time of registration.
