Arizona state law requires immunization of all students in grades K-12. A complete immunization history must be furnished and on file or students will be subject to expulsion according to state law (R9-6-501-504). All students new to Arizona schools must show documentation with the date and type of each dose administered or laboratory evidence of immunity. Parental recall of dates and types of immunizations received will no longer be acceptable.

Arizona Administrative Code R9-6-701 (Required Immunizations for School Attendance) has been amended to include Hepatitis B in the list of diseases for which children must be immunized. Additionally, a second Measles, Mumps, and Rubella (MMR) vaccination was added to the required immunization schedule.

The immunization requirements for Hepatitis B and the second MMR applies to children in kindergarten and first grade. This rule requires these additional immunizations of any child entering kindergarten and first grade in Arizona, unless that child has previously been fully immunized for these diseases. These immunization requirements are defined on the Arizona Immunization Program Office Web page.

The Hepatitis B series does not have to be completed prior to Kindergarten and first grade entry, but must be initiated prior to entry and completed according to schedule. The recommended time schedule for Hepatitis B administration is as follows: The 2nd dose shall be received at least one month after the 1st dose, and the 3rd dose shall be received at least two-four months after the 2nd dose.
